Many patients in neuro rehabilitation require a multi-disciplinary team generally consisting of a physical therapist, occupational therapist, and speech/language pathologist to address difficulties that can occur as a result of neurological injury or disease. Neurologic injuries may affect a patient's ability to move, feel touch, care for themselves, or talk.
Patients are referred to us in outpatient from a number of sources, including other inpatient rehabilitation programs, physicians and insurnace companies. Once the patient goes home from an acute care hospital, he/she may need to continue therapy on an outpatient basis.
Physical therapy, to help them with issues of mobility such as walking, using a wheelchair, walker, or a cane.
Occupational therapy, to help the patient regain strength needed for activities of daily living such as dressing, eating, grooming, showering, bathing, and preparing meals.
Speech therapy, to help the patient with speaking, reasoning, judgment, memory issues, swallowing, and safety.
A physician prescription may be dropped off at our office or faxed using the information below. Once the prescription is received, our referral coordinators will request financial clearance from your insurance company and will call you as soon as possible to schedule your appointment.
Medi-Cal patients require a Treatment Authorization Request (TAR) that is sent to the Medi-Cal office for authorization. This process may take a few weeks.
Once the authorization has been received from Medi-Cal, we will call you to set up an appointment.
Our office is open from 7:30 a.m. to 7:30 p.m. to accommodate patients and family members who need appointments before or after work, school, or during a lunch hour.
