How to start your request for a copy of your Medical Records at LLUMC
Obtaining a copy of your LLUMC medical record is easy. To start your request, simply download, print, complete, and sign the following form. Fax or mail them back to us, to the attention of the Health Information Management Office.
Please be sure to sign the form. Unsigned requests cannot be processed.

Your request will be processed and fulfilled within 15 business days from the day it is received. Please allow reasonable time to process your request. We will either mail it to the address specified on the authorization form, or you may pick them up at our office if you make arrangements with the release of information staff.
Records can be picked up between the hours of 8:00 a.m. and 5:00 p.m. Monday - Thursday and Friday from 8:00 a.m. - 3:30 p.m.
There is a 25 cent per page copying fee.
