IVF SERVICE PACKAGE CHARGES Effective April 1, 2008
- The package rate is a cash flat rate (cannot be itemized ) and does not include the cost of medications, supplies or additional procedures, unless specified. Patients who do not have insurance coverage for any of the procedures included in the package are eligible for the discount program. Normally this package totals $11,422. per IVF cycle. We are pleased to offer this package for $7,900. which represents a 31% discount ($3,522).

IVF CYCLE
- $1,828.-3,200. Cost ranges for ovulation induction. Ovulation induction is not included in package pricing and payment is due with each visit. Ovulation induction begins with the first day of FSH injection or the first day of Lupron injection in "Flare"cycles and ends with the HCG injection. Ovulation induction averages $2,000. per cycle. During ovulation induction, combinations of the following charges should be expected.

- Payments may be made during normal office hours using Master Card, VISA, currency or a cashier's check. Personal checks may be accepted for amounts less than $1,000.
- Separate checks may be written for embryo freezing and assisted hatching. Your checks will be returned to you or shredded if these services are not provided.
- Prices quoted are good for ninety days from the following date marked.
