Necessary items/information:
-Insurance card (front and back)
(Medi-Cal patients must have full scope, NOT emergency)
-Most recent echocardiogram
-Operative reports of cardiac surgery
-Heart catheterization/coronary angiogram reports
-Blood tests
CBC, comprehensive metabolic panel, blood/urine cultures
-History/physical
-Recent discharge summary
-Any other pertinent medical conditions

There must be a doctor-to-doctor discussion with a mutually determined plan, bed availability, and insurance authorization prior to transfer.
